Fazal Khan, MD

Sanjeeda Jabeen, MD, is a board-certified Internal Medicine physician who earned her medical degree from Gandhi Medical College in India in 2010. She completed her residency at Aurora Health Care in Milwaukee, Wisconsin, in 2019. She treats a variety of general medical conditions, including hypertension, hyperlipidemia, diabetes mellitus type 2, and hypothyroidism, with a focus on preventive medicine. Dr. Jabeen joined Kelsey-Seybold in August 2024.

Rachel Alford, FNP-C, first completed a Bachelor of Science in Nursing University of Texas Health Science Center at San Antonio School of Nursing in 2014. After caring for patients as a registered nurse for several years, she went on to the University of Texas at El Paso School of Nursing to complete her Master of Science in Nursing in 2021. Rachel also holds a Bachelor of Science in human nutrition from the University of Houston. She joined Kelsey-Seybold Clinic as a family medicine nurse practitioner in April 2025. With a holistic approach to care, Rachel provides primary care for patients of all ages. She offers well checkups and sick care for acute illness and chronic conditions such as hypertension, diabetes, and hyperlipidemia.

Omada Omosebi, MD, earned her medical degree from St. Matthew's University College of Medicine in the Cayman Islands in 2013. Dr. Omosebi then completed her Family Medicine residency at the University of Kentucky's East Kentucky Family Medicine Residency Program in 2019. She has extensive experience managing acute illnesses, chronic disease care, and preventive health. Her clinical interests include geriatric care and supporting her patients' quality of life through a collaborative approach to managing both acute and chronic conditions. Dr. Omosebi values building strong, caring relationships with patients and their families and is dedicated to clear, open communication. She joined Kelsey-Seybold Clinic in November 2024.
