With more than 26 years of experience, Transitions is a community-based private, nonprofit organization that provides therapy, advocacy and support services for patients with mental disorders and psychiatric illness. It is a recovery-based organization that promotes and enhances the health and well-being of individuals, families and the community in which they live. The organization offers short-term individual and group consulting services for individuals seeking assistance in understanding, responding to and working with patients demonstrating challenging behaviors, personalities and perspectives. It additionally provides housing, supported employment, vocational training, counseling, education, mental assessment and rehabilitative services. The organization is a member of the National Alliance on Mental Illness and Commission on Accreditation of Rehabilitation Facilities. Transitions is located in Rock Island, Ill.
