Founded in 1999, Countryside Hospice is a nonprofit health care provider that is certified by Medicare, Medicaid and The Joint Commission in the Southeast. It has a staff that works with patients, families, physicians, hospitals, discharge planners and nursing homes. Countryside Hospice has a team that includes physicians, registered nurses, home health aides, social workers, therapists, chaplains and pastoral counselors and volunteers. It provides hospice care services for cancer, liver, pulmonary and cardiac diseases, renal failure, HIV, stroke and coma. Headquartered in Anniston, Ala., Countryside Hospice also maintains locations in Sylacauga and Roanoke, Ala., and Newnan and LaFayette, Ga.
